OSI Physical Therapy has a long history of delivering high quality therapy care and excellent customer service for our patients and referral sources in the east metro area – 34 years to be exact. Paul Niemuth, DPT, now a professor at The University of St. Catherine’s Physical Therapy School, started OSI in 1979 and immediately focused time and financial resources on clinical education and professional development. In those early years, Niemuth and Mike Ripley led OSI’s team to create a culture of quality care and customer service that has never waned.
OSI has worked collaboratively with St. Croix Orthopaedics and many other specialty and primary care physicians over our 30+ years to develop programs and deliver care that results “Triple Aim Value”– measurable quality, an exceptional patient experience, and lower total cost of care. “Quality” in therapy is all about improving patient’s function – ability to move and do activities better and with less pain. Sports, recreation, work, and normal day-to-day activities.
And we can PROVE that OSI’s therapists provide our patients with tremendous Value – functional improvement per visit – that exceeds a vast majority of clinics around the country.
Since 2005, OSI and all of the other independent therapy practices within the Therapy Partners’ network have used a scientifically researched, risk adjusted outcomes measurement software called FOTO – Focus on Therapeutic Outcomes – to prove the high value of our services. FOTO is used by over 3000 clinics and 10,000 therapists around the country; has over 5 million patient cases in its data base; has been analyzed and proven reliable and valid with over 80 scientific studies; and is accepted by the US government’s Centers for Medicare and Medicaid Services (CMS).
How Does FOTO Work?
Before their first therapy visit, patients complete a FOTO Intake Functional Survey. Patients answer computer-based questions about how their injury or condition affects their functional activities such as walking, lifting, carrying, reaching, running, and other things that are specifically important to each patient. FOTO factors in the patient’s age, severity of injury, other medical conditions, and other ‘risk factors’ and then scores the patient’s responses, compares to the tens of thousands of other people who have had a similar situation, forecasts the functional outcome, and predicts the number of visits necessary to achieve that outcome.
During the course of therapy care, patients complete FOTO Status Surveys that shows their improvement in function. Patients complete a final survey on their last therapy visit for the final comparison to their FOTO Intake.
Upon the completion of therapy care, FOTO scores each individual patient’s functional improvement and number of visits, and assigns the patient a Value Outcome of “Higher than Expected” “Expected” or “Lower then Expected” depicted in the FOTO Outcomes 9 Cell Value Matrix. Green Cells (1,2,4) indicate Higher than Expected Value with highly improved function in lower than expected visits. Yellow Cells (3,5,7) indicate Expected Value related to function and cost. Red Cells (6,8,9) indicate Lower than Expected functional improvement coupled with higher than expected number of therapy visits.

OSI Physical Therapy patient outcomes among highest in the nation. Better Patient Care. First and foremost, OSI uses FOTO to improve our patient care.
- We look at each individual therapist’s outcomes to determine their areas of strength and need for improvement. Therapists with high outcomes for specific conditions serve as mentors; those with lower outcomes work with mentors and seek educational experiences to improve in their areas of need.
- We try to direct patients to the therapists who get the best results for the patient’s specific condition or injury.
- Therapists use FOTO’s prediction of expected functional outcomes to help patients set realistic expectations.
- FOTO helps therapists address each patient’s specific functional needs and goals
- FOTO helps therapists accurately predict how many visits each patient needs
- Each OSI therapist tries to achieve excellent results in fewer visits – lower costs to patients and their insurance company.
- FOTO helps therapists determine when a patient is ready to discontinue therapy care; this helps control costs and manage care effectively.
Health Plan Contracts. We also use FOTO to establish provider contracts that reward us for improved patient care and outcomes. This creates incentives for our clinics to do BETTER, rather than do MORE.
Collaboration with Other Providers. FOTO outcomes allow us to more effectively work with other physician groups to develop better care models and measure the results of those new models.
OSI FOTO Outcomes. Overall, OSI therapists and clinics achieve excellent function and cost outcomes (value) compared to clinics around the country. OSI’s most recent functional and cost outcomes are in the 86th percentile nationally, and OSI’s average number of visits per person receiving therapy is 33% lower than national average – 8 visits compared to 12 visits. Some OSI clinics and therapists are in the 90th percentile.
